- Section 1: When to Use N12 form
- Reason 1: Landlord, family member, or caregiver wants to move in for at least one year.
- Reason 2: Purchaser, family member, or caregiver wants to move in, and:
- Complex has less than 3 units, and
- Agreement of purchase and sale for the complex exists.
- Cannot use it if certain conditions regarding condominium conversion or severance apply.
- Section 2: Information about Compensation
- Must pay tenant one month’s rent as compensation or offer an acceptable alternative rental unit.
- Compensation must be paid before the termination date.
- Section 3: How to fill the N12 form
- To and from: Fill in tenant’s name(s) and landlord’s name(s).
- Address: Provide complete rental unit address.
- Termination Date: At least 60 days after the date giving tenant this notice, must be the last day of a rental period or fixed term.
- Tenants can end the tenancy earlier with 10 days notice.
- Shade circle next to reason for ending tenancy and box next to who intends to move in.

- Include name, phone number, and date of signing.
- Representative’s information required if applicable.
